Pinching the return line isn't always enough. The injectors need a good bit of pressure to fire. If you apply a little bit of air pressure to the return line, and crank it, that will be your best chance of getting it to fire up quicker. The cam sensor is only used as a reference for the ECM so it knows which cylinder is at TDC. I would take a look at the crank sensor as well.
